原因
  • ・日本国内ユーザー向けの Exchange Onlineインフラストラクチャーの一部で、リソースの消費が予想以上に増加しました。
  • このため、DNS トラフィックが処理のしきい値を超え、サービスに影響が発生しました。
  • ・この問題の根本的な原因は現在も調査中であり、詳細については、最終的な PIR(Post – Incident Report)にて報告されます。

このため、影響を受けるインフラストラクチャーに対して、これらの緩和措置を実施し、サービスのモニタリングの結果、影響が改善されたことを確認しています。

Root Cause

  • Processing components were not performing within optimal performance thresholds because of a legacy process that required tokens to be processed on specific components.
  • In isolation this process wasn’t problematic, but combined with the large number of requests, this resulted in resource saturation, causing impact across multiple Microsoft 365 apps

Actions Taken (All times UTC)

Thursday, December 1
  • ・11:41 PM – We received alerts that Exchange Online components had dropped below standard operational thresholds, causing impact for end users.
  • We started a high-priority investigation to understand what had happened and establish a fix.
Friday, December 2
  • ・6:00 AM – After extended monitoring through peak traffic we confirmed that the issue is fully mitigated.
  • We begin returning the remaining infrastructure back into active service. We also continue our investigation into the underlying cause of the issue.
  • ・12:30 AM – We correlated reports from support that impact may not be limited to Exchange Online, but also Microsoft Teams and Microsoft Power Apps. We continued analyzing diagnostic logs.
  • ・12:52 AM – We restarted a section of affected infrastructure to see if this would provide relief.
  • ・1:26 AM – We suspected that a token-issuing service may have reached an operational threshold, causing impact for users hosted in APAC.
  • We started analyzing the token issuer service, which was restarted as part of our actions at 12:52 AM. Additionally, after the restarts, we saw improvement with the service.
  • ・1:31 AM – We noticed that recovery had stalled and started to explore further recovery options.
  • ・3:14 AM – We restarted services on more affected infrastructure, specifically Client Access Front-end (CAFÉ) infrastructure, to make sure that all services come back online in a healthy state.
  • ・3:43 AM – We saw service health improving at this time, for all impacted services.
  • ・5:27 AM – Microsoft Teams reported that their services had recovered.
  • ・5:57 AM – Recovery for Exchange Online was progressing slowly.
  • ・6:00 AM – We narrowed down the cause to two specific services. We started analyzing logs from thos services to understand why the impact scenario cascaded so broadly.
  • ・7:02 AM – For the root cause investigation, we discovered that a specific app caused a spike in token requests at approximately 12:00 AM.
  • This spike happened for an app which requires CAFÉ arrays to authenticate, and the increase (of approximately 40 times) may have put the database into contention.
  • ・7:45 AM – After a period of analysis, we discovered that a legacy process requires tokens are processed on Domain Controllers, but because of the volume of requests, these DCs became degraded, resulting in impact.
  • We developed an Encryption Control System (ECS) fix for the affected app, which would disable the legacy process. We tested this in a pre-production environment before applying it to the live production environment.
  • ・8:27 AM – We started testing the ECS fix.
  • ・9:14 AM – In parallel, we restarted any affected sections of infrastructure that were still showing signs of degradation.
  • ・9:54 AM – The restarts were approximately 50% complete, and the ECS fix had completed deployment. We started a period of monitoring to make sure the service had fully recovered.
  • ・10:13 AM – We confirmed that the service was healthy and declared the incident resolved.

  • ・More info : Impact includes the following services but is not limited to :
  • – Exchange Online
  • – Microsoft Teams (Mitigated)
  • – SharePoint Online
  • – Microsoft PowerApps
  • – Microsoft PowerAutomate
  • Microsoft Teams messaging, chat, channels, and other core services do not appear to be affected at this time.
  • For Exchange Online specifically : – Users may be unable to access the service
  • – Users may be unable to send/receive mail/files
  • – Users may be unable to leverage general features
  • For Microsoft Teams specifically : – Users may experience issues scheduling/editing meetings and/or live meetings
  • – Users may be unable to search Microsoft Teams
  • – Users may be unable to load the Assignments tab in Microsoft Teams
  • – Users may be unable to use the Template Management or People Picker features of the Microsoft Teams Approvals app
  • – Users may not be able to load their contacts including favorites and user created contact groups
  • – Users may not be able set or see actual presence
  • – Users may not be able to initiate new chat
  • – Users may be unable to see the deleted channels under a team

  • Based on our understanding of the issue, users may be unable to use any feature within Exchange Online or Microsoft Teams.
  • ・The Exchange Online features may include : – Users may be unable to access the service
  • – Users may be unable to send/receive mail/files
  • – Users may be unable to leverage general features
  • ・For Microsoft Teams specifically : – Users may experience issues scheduling/editing meetings and/or live meetings
  • – Users may be unable to search Microsoft Teams
  • – Users may be unable to load the Assignments tab in Microsoft Teams.
  • ・Current status : Our initial investigation indicates that there our service infrastructure is performing at a sub-optimal level, resulting in impact to general service functionality.
